How your intake information is handled
What we collect
We collect the answers you choose to provide, your contact information, your service preferences, and a record of which approved questions were shown. Please do not include unnecessary medical, financial, legal, or other highly sensitive information.
How it is used
Your answers are securely stored for Lizette to review before a possible consultation. They help create a non-diagnostic service-fit summary and do not automatically accept you into services.
Use of OpenAI
OpenAI’s API processes relevant answers to select approved follow-up questions and generate tentative summaries. Submitted information is not used to train a public or shared AI model. User-entered instructions cannot override the questionnaire’s safety rules.
Storage and retention
Intake records are kept in protected platform storage and are not publicly accessible. Before account creation, progress and completed results are connected to a secure browser token and retained for up to 30 days. Once claimed by a verified client account, the normal private-record retention period applies. Lizette can delete an intake sooner upon request.
Your choices
Optional questions may be skipped. You may ask Lizette to review, correct, or delete your submitted intake by emailing lelainew@yahoo.com.
Important limitations
The questionnaire is not medical care, therapy, diagnosis, crisis support, or a substitute for a licensed professional. If you may be in immediate danger, call 911 or your local emergency number. In the United States, call or text 988 for crisis support.
